The Social Services Designee/Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

Responsibilities:
- Meeting the resident/guest on or shortly after the admission and following them until they are discharged to the community.
- Attend the 72-hour care plan meeting.
- Confirm contact information on the face sheet.
- Ordering or recommending resources to acquire durable medical equipment as needed/recommended by therapy for mobility or nursing to attain optimal health status and ordered by practitioner.
- Arrange home health care for after discharge with the resident/guest's preferred provider, as soon as possible.
- Ordering and coordinating IV home infusion services, wound vacs, trach supplies, tube feeding supplies, vents and vent supplies, if necessary.
- Coordinating nursing education with the proposed caregiver.
- Overall coordination of the discharge.
- Attending care conferences.
- Attending Medicare Meetings.
- Any item related to the discharge process for residents who will be going home.
- Ensuring completion of the Post Discharge Plan of Care and Summary.
Education and/or Experience:
- High school diploma/GED.
