The Central Intake Coordinator is responsible for managing and coordinating the referral and admission process for residents across the nursing home company’s facilities. This role serves as the primary point of contact for hospitals, physicians, families, and referral partners, ensuring timely and accurate processing of admissions while maintaining excellent customer service. The Central Intake Coordinator supports census development goals by ensuring a smooth transition for residents and their families from referral through admission.

Responsibilities:
* Receive, review, and process all incoming referrals for multiple facilities.
* Verify clinical, financial, and insurance information to determine admission eligibility.Collaborate with clinical teams to assess care needs and placement
* Serve as the central liaison between referral sources, facility admissions teams, and families.
* Communicate regularly with hospitals, case managers, physicians, and other referral partners.
* Provide timely updates to families regarding admission requirements, documentation, and placement status.
* Ensure all pre-admission paperwork, insurance authorizations, and medical records are complete and accurate.
* Maintain compliance with federal, state, and company standards for admissions documentation.
* Accurately enter and track referrals and admissions in the company’s database or CRM system.
Requirements:
- High school diploma or equivalent required; associate’s or bachelor’s degree in healthcare, business, or related field preferred.
- Experience in healthcare admissions, intake, or case management (nursing home or post-acute care experience preferred).
- Knowledge of Medicare, Medicaid, and private insurance processes.
- Proficiency in electronic medical record (EMR) systems and Microsoft Office Suite.
